How to notify us

Click on the link to access the form. There are then two options to return the completed forms to us:

  1. Complete the form electronically – this pdf can be typed into. Save the completed form to your computer and send it to SWCAR as an attachment using NHSmail. Please note that we will only accept completed forms if sent to our NHSmail address (phe.swcar@nhs.net). You need to use an NHSmail account (@nhs.net) for this, for the transfer of confidential information to be secure.

  2. Print out the form, fill it in by hand and return to us by post to this address:
    SWCAR, National Congenital Anomaly and Rare Disease Registration Service (NCARDRS), CKO, PHE, 3rd Floor, 2 Rivergate, Bristol, BS1 6EH

Tips on using the form

The form incorporates antenatal and postnatal information. Therefore feel free to only complete the part of the form that is relevant to your involvement with the case. For instance, if you are an antenatal coordinator, complete the mother information and the antenatal investigations sections. If you are a paediatrician, complete the baby/child information and postnatal details of the anomaly and as much as you know about the mother and her antenatal history.
